Job summary

Overland Park Regional Medical Center in Lenexa, Kansas seeks a Registered Nurse Case Manager to guide patients and families through the care journey, coordinate services, and develop individualized care plans with physicians and the care team.

This role emphasizes discharge planning, psychosocial assessments, education for patients and families, and collaboration with the Rehab Program Director and interdisciplinary team to ensure safe transitions and high-quality care.

Qualifications

• Associate Degree in Nursing is required.
• Bachelor’s Degree in Nursing is preferred.
• 3+ years experience in clinical nursing is required.
• InterQual experience is preferred.

Responsibilities

• Guide patients and families through program orientation and discharge planning.
• Coordinate education for patients, families, and caregivers.
• Develop and implement individualized treatment plans reflecting patient needs.
• Complete psychosocial assessments and communicate findings to the care team.
• Collaborate with Rehab Program Director and interdisciplinary team for seamless care.
• Promote HCA Healthcare values in patient and family interactions.
