Job summary

Duet Nebraska is seeking an experienced Registered Nurse Case Manager to oversee clinical nursing care, assessment, planning, and coordination for people supported. This role requires leadership in implementing the nursing process, ensuring safety, quality, and adherence to policies, with supervision from the Director of Nursing.

The RN Case Manager will manage medication administration, provide nursing leadership, and collaborate with a multidisciplinary team to meet health and habilitation

Qualifications

• Graduate of an accredited School of Nursing and Nebraska RN license required.
• RN licensure in Nebraska or compact state with Nebraska eligibility required.
• Ability to pass criminal background and health screenings.

Responsibilities

• Provide professional nursing care following the nursing process to people supported.
• Assess, plan, implement, and evaluate individualized care plans.
• Delegate and monitor care with unlicensed staff and medication aides as needed.
• Lead and participate in ISP meetings and nursing reviews as required.
• Maintain thorough documentation of medical issues, plan of care, and outcomes.
• Communicate with physicians, vendors, social workers, and care teams to ensure quality care.
