Job summary

Qualifications

• Ability to pass a criminal background and background checks; valid driver’s license.

Responsibilities

• Assess, plan, implement, and evaluate nursing care for people supported.
• Coordinate health and habilitation supports with the ISP team.
• Delegate nursing tasks to unlicensed staff and monitor interventions.
• Maintain thorough, legible medical documentation and HIPAA compliance.
• Communicate with physicians, vendors, social workers, and team members as needed.
