Duet Nebraska is seeking an LPN Case Manager to deliver high-quality nursing care in community-based settings. Under the Director of Nursing, you will implement the nursing process, coordinate health supports, and oversee medication-related activities by aides to ensure safety and continuity of care.

You will assess health needs, collaborate with the RN Case Manager and ISP team, and train direct support staff on procedures and equipment while maintaining thorough documentation and adherence to

Formación

• Graduate of an accredited School of Nursing and Nebraska LPN licensure is required.
• Minimum two years’ experience as an LPN preferred.
• Knowledge of nursing theory, quality standards, procedures and ethics.

Responsabilidades

• Provide nursing care using the nursing process and ensure health and safety of individuals.
• Monitor and deliver health and habilitation supports to individuals.
• Coordinate medication administration with aides and other health professionals.
• Delegate and supervise direct care staff to implement care plans.
• Document nursing assessments, observations, and plan-of-care updates.
• Communicate with physicians, vendors, social workers, and ISP team as needed.
• Attend hospital discharge meetings and essential agency training.
• Promote a positive, supportive environment for individuals and staff.

Educación

Graduate of an accredited School of Nursing and State of Nebraska LPN licensure
