Qualifications

• Active Registered Nurse License in Illinois in good standing.
• Certification in Case Management preferred.
• Current CPR (BLS) certification must be maintained.

Responsibilities

• Manage caseload and complete case management attestations and assessments.
• Document, communicate, and coordinate care plans across settings.
