Job summary

Duet in Nebraska is seeking an LPN Case Manager to deliver professional nursing care, monitor health supports, and coordinate services for individuals in home and day programs. Under the Director of Nursing, you will assess, plan, implement, and evaluate care, supervise direct support staff, document activities, train staff, and liaise with physicians and vendors.

Qualifications

• Nebraska LPN license required.
• Minimum two years’ experience as an LPN.
• Ability to work with individuals with developmental disabilities.
• Pass criminal background check and do not appear on APS/CPS registry for abuse/neglect.

Responsibilities

• Reports to work as scheduled.
• Provide nursing care using the nursing process.
• Assess, plan, implement and evaluate health plans with the RN Case Manager.
• Delegate and supervise direct care staff per care plans.
• Maintain thorough documentation of medical issues and nursing interventions.
• Act as medical liaison and train staff in nursing procedures.
• Communicate with physicians, vendors, social workers, and family members as needed.
• Attend hospital discharge meetings and required ISP meetings.
