Job summary

AccentCare RN Case Manager position in Gurnee, IL, covers the care plan from admission through discharge. You will lead direct and indirect nursing care, coordinating with LPNs, CHHAs, and allied staff to meet patient goals.

Qualified candidates hold an RN license in Illinois with at least one year of RN experience and CPR certification, and prefer strong assessment and documentation skills for timely progress notes.

Qualifications

• Graduate from an approved school of professional nursing and licensed to practice as an RN in Illinois.
• Minimum of one year of RN experience.
• Current CPR certification required.

Responsibilities

• Assess nursing needs and manage the patient’s care plan from admission to discharge.
• Complete admission assessment including patient history.
• Coordinate interdisciplinary care and communicate with physicians.
• Review medications for safety, allergies, and interactions.
• Document progress, discharge notes, and required paperwork timely.
