Job summary

AccentCare in Gurnee, IL is seeking a registered nurse case manager to oversee patient care plans from admission through discharge and ensure quality care. The role combines direct and indirect patient care and coordinates with nurses, LPNs, and CHHAs to implement the plan.

You will conduct initial assessments, monitor medications, communicate with physicians, and document progress. Requires IL RN license, CPR, and at least 1 year of RN experience.

Qualifications

• Graduate from an approved school of professional nursing and licensed to practice as an RN in IL.
• Minimum of one year of RN experience.
• Current CPR Certification required.

Responsibilities

• Assessment of nursing needs and overall management of the patient’s care plan.
• Complete initial patient assessment on admission, including patient history.
• Identify appropriate nursing diagnoses and monitor meds for adverse reactions and contraindications.
• Establish measurable goals aligned with diagnosis and inform physician of results and interventions.
• Implement nursing interventions within the Nurse Practice Act and scope of practice.
• Re-evaluate patients’ needs as required and document progress and discharge notes in a timely fashion.
