Job summary

AccentCare in Lombard, IL is seeking an RN Case Manager to manage patients' care plans from admission through discharge and ensure delivery of quality patient care. The role combines direct and indirect patient care and directs staff nurses, LPNs, and CHHAs to meet each patient's plan of care.

Qualifications include graduating from an approved nursing program, having an active Illinois RN license, at least 1 year of RN experience, and current CPR certification.

Qualifications

• Must be a graduate from an approved school of professional nursing.
• Minimum of one year of RN experience.
• Current CPR certification required.

Responsibilities

• Assess nursing needs and manage the patient care plan.
• Complete initial patient assessment upon admission and collect patient history.
• Select nursing diagnoses and coordinate interventions with the physician.
• Evaluate meds for possible drug therapy issues and adverse reactions.
• Establish realistic goals aligned with diagnosis and care plan.
• Inform physician of assessment results and recommended interventions.
• Implement nursing interventions within the Nurse Practice Act and established goals.
• Re-evaluate patient needs as required and adjust plan of care.
• Document assessments, progress, and discharge notes in a timely manner.
