Job summary

AccentCare, Inc. is seeking a registered nurse case manager to oversee patient care plans from admission through discharge in Edina, Golden Valley and surrounding areas.

This role blends direct and indirect patient care while coordinating nursing staff to deliver each patient's plan of care. You will assess nursing needs, perform initial assessments, monitor medications, and communicate with physicians about interventions.

Qualifications

• RN license in the state of operation.
• At least 1 year of RN experience.
• Current CPR certification required.

Responsibilities

• Assess the status of nursing needs and manage the patient's care plan.
• Complete initial patient assessment upon admission, including history.
• Select appropriate nursing diagnoses and monitor meds for adverse reactions.
• Inform physician of assessment results and recommended interventions.
• Implement nursing interventions and re-evaluate patient needs as required.
• Document assessments, progress and discharge notes in a timely fashion.
