Job summary

AccentCare, Inc. is seeking a Registered Nurse Case Manager for Home Health in the Minnesota area. The role covers Edina, Golden Valley, and surrounding communities, with a Mon–Fri 8:00 a.m.–5:00 p.m. schedule and a total annual pay of $87,000–$97,000.

A $5,000 sign-on bonus is offered. The position involves managing patient care plans from admission to discharge and coordinating a care team. The RN will supervise direct and indirect care, assess nursing needs, and ensure quality patient care

Qualifications

• Graduate from an approved school of professional nursing
• Minimum of one year of RN experience
• Current CPR Certification required

Responsibilities

• Assess nursing needs and manage the patient's care plan
• Complete initial admission assessment with patient history
• Select appropriate nursing diagnoses
• Assess medications for interactions, allergies and adverse reactions
• Establish measurable goals aligned with diagnosis
• Inform physician of assessment results and interventions
• Implement nursing interventions per diagnosis and goals
• Re-evaluate patient needs as required
• Document assessments, progress, and discharge notes timely
• Inform physician of changes in patient condition
