Job summary

AccentCare, Inc. is hiring an RN Case Manager in Franklin, TN. The role covers patient care plans from admission through discharge and directs nursing staff to deliver the plan of care, coordinating with allied health clinicians as needed.

The ideal candidate holds an RN license in Tennessee, has at least 1 year of RN experience, and current CPR certification. The position includes a Monday–Friday schedule with on-call duties and compensation that reflects a per-point pay structure.

Qualifications

• Graduate from an approved school of professional nursing and currently licensed as an RN in the state of operation.
• Minimum of one year of RN experience.
• Current CPR Certification required.

Responsibilities

• Assess and manage the patient’s nursing needs and overall care plan.
• Complete initial patient assessment upon admission and collect history.
• Identify nursing diagnoses and coordinate interventions with physicians.
• Review medications for potential adverse reactions and allergies.
• Document assessments and discharge notes in a timely manner.
• Communicate changes in patient condition to the physician and team.
