Job summary

AccentCare, Inc. is seeking an RN Case Manager to oversee care plans from admission through discharge in Warren & Claiborne Counties, MS. The role includes directing staff and coordinating with a multidisciplinary team to ensure quality patient care.

The position requires MS RN licensure, CPR certification, and at least 1 year of RN experience. Schedule is M-F 8am-4:30pm with on-call duties; compensation ranges from $71,000 to $79,000 plus a $5,000 bonus.

Responsibilities

• Assess nursing needs and manage the patient care plan
• Complete initial patient assessment upon admission with patient history
• Select appropriate nursing diagnoses
• Assess medicines for adverse reactions and contraindications
• Set realistic, measurable goals aligned with diagnosis
• Inform physician of assessment results and interventions
• Implement nursing interventions within Nurse Practice Act
• Re-evaluate patient nursing and clinical needs as required
• Document assessments, progress, discharge notes timely
• Inform physicians of changes in patient condition
