AccentCare, Inc. is seeking a Registered Nurse (RN) Case Manager to coordinate patient care from admission through discharge in Rankin County, MS. This full-time position combines direct and indirect care and guides RNs, LPNs, and CHHAs to implement individualized plans of care.

The role involves assessing nursing needs, documenting progress, and communicating with physicians to adjust interventions. Schedule is Monday to Friday, 8:00 am–4:30 pm, with on-call duties and a pay band of

Qualifications

• Graduate from an approved school of professional nursing and licensed RN in the operating state.
• Minimum of one year of RN experience.
• CPR certification required.

Responsibilities

• Assess nursing needs and manage patient care plans.
• Complete initial patient assessment at admission with history.
• Select appropriate nursing diagnoses and monitor progress.
• Review medicines for potential adverse reactions or allergies.
• Set realistic, measurable goals aligned with diagnosis.
• Inform physician of assessments and recommended interventions.
• Document assessments, progress, and discharge notes timely.
