Job summary

UP Health System - Marquette is hiring a Registered Nurse (RN) Case Manager to join our Acute Care team. You will oversee patient care coordination, discharge planning, and collaboration with physicians and care teams to ensure timely discharges and appropriate care levels.

The role requires a Michigan RN license and an Associate degree in nursing (BSN preferred), with at least five years of hospital nursing experience and BLS-HCP certification.

Qualifications

• Current Michigan RN license.
• Associate degree from accredited nursing school (bachelor’s preferred).
• Basic Life Support (BLS-HCP) certification.
• Five years nursing experience (hospital preferred).

Responsibilities

• Monitors and manages patient care to promote continuity of care, optimal outcomes, patient satisfaction, cost efficiency, and compliance.
• Consults with nursing staff and multidisciplinary team to evaluate patient status and appropriateness of medical care, including admission, length of stay, transfer and discharge.
• Monitors patient and family satisfaction; responds to questions and complaints from patients, families, and payors regarding care.
• Participates in discharge planning including coordinating patient transfers to other facilities and coordinating community resources; provides discharge education and resource referrals.
• Performs chart review to identify issues with service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.

Education

Associate degree in Nursing
