Job summary

UP Health System - Marquette is seeking a Registered Nurse Case Manager to coordinate patient care, monitor outcomes, and support timely discharges across acute care.

You will collaborate with physicians and multidisciplinary teams, conduct chart reviews, and participate in discharge planning and resource referrals.

The role requires Michigan RN licensure, an associate degree (bachelor's preferred), BLS, and at least five years of nursing experience in a hospital setting.

Qualifications

• Current Michigan RN license
• Associate's degree in nursing (Bachelor's degree preferred)
• Basic Life Support (BLS-HCP)
• Five (5) years nursing experience (hospital preferred)

Responsibilities

• Monitors and manages patient care to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance.
• Consults with nursing staff and multidisciplinary team regularly to evaluate patient status and appropriateness of medical care, including admission, length of stay, transfer and discharge.
• Monitors patient and family satisfaction and responds to questions and complaints from patients, family members, and payors regarding care.
• Participates in discharge planning including coordinating patient transfers to other facilities and coordinating community resources.
• Performs chart review to identify issues with service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.

Schedule: 10 x 8-hour Day shifts or 8 x 10-hour Day Shifts/pp, Flexible Start Times, Every 4th Weekend and two (2) Holidays

Our Acute Care RN Case Managers are essential partners in patient flow and clinical outcomes. Using expert clinical assessment, real-time collaboration with physicians and care teams, and strong utilization management skills, they ensure patients receive the right level of care while supporting timely discharges, reducing avoidable delays, and improving the overall hospital experience.

What we\'re looking for

Applicants should have a current Michigan RN license and possess an associate\'s degree from an accredited nursing school (bachelor\'s preferred).
