Job summary

Munson Healthcare in Michigan seeks a Hospital-based Case Manager to coordinate patient care, ensuring continuity with compassion and reliability. You will work with physicians to evaluate, educate, and refer patients for the next level of care, while coordinating placement and a safe discharge.

Required active Registered Nurse licensure in Michigan and at least 3 years of progressive clinical experience (including 2 years as an RN); Case Management Certification is preferred and may be obtained

Qualifications

• Current licensure as Registered Nurse in Michigan.
• Minimum 3 years progressive clinical experience, including 2 years as RN.
• Case Management Certification preferred; may be obtained within 2 years.

Responsibilities

• Coordinate and manage patient care to ensure continuity with compassion and reliability.
• Collaborate with physicians to evaluate, educate, and refer patients for next level of care.
• Coordinate placement and discharge planning with ancillary services.
