Job summary

UP Health System – Marquette is seeking a Registered Nurse (RN) Case Manager to oversee patient care coordination and discharge planning in a busy hospital setting.

The role requires Michigan RN licensure and an associate’s degree in nursing, with a bachelor’s degree preferred. This is a full-time position with a day shift pattern and involvement in quality of care initiatives.

Qualifications

• Current Michigan RN license.
• Associate’s degree from an accredited nursing school; bachelor’s degree preferred.

• Monitors and manages patient care to promote continuity of care, outcomes, satisfaction, and cost efficiency.
• Consults with nursing staff and multidisciplinary team on admission, length of stay, transfer, and discharge decisions.
• Monitors patient and family satisfaction; answers questions from patients, families, and payors.
• Participates in discharge planning and coordinates transfers and community resources.
• Performs chart reviews to identify issues with service delivery, outcomes, and reimbursement.

Registered Nurse (RN) Case Management

Job Type: Full-Time | 1.0 FTE or 80 hours per person | Monday–Friday

Schedule: 10 x 8‑hour day shifts or 8 x 10‑hour day shifts per person, flexible start times, every 4th weekend and two holidays.

Responsibilities

• Monitors and manages patient care to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance.
• Consults with nursing staff and multidisciplinary team regularly to evaluate patient’s status and appropriateness of medical care including admission, length of stay, transfer, and discharge.
• Monitors patient and family satisfaction. Responds to questions and complaints from patients, family members, and payors regarding care.
• Participates in discharge planning including coordinating patient transfers to other facilities and coordinating community resources. Provides discharge education and resource referrals to patients.
• Performs chart review to identify actual or potential issues with service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.

Qualifications

Applicants should have a current Michigan RN license and possess an associate’s degree from an accredited nursing school (bachelor’s preferred).

Additional Requirements

• Basic Life Support Health Care Provider (BLS‑HCP)
• Five (5) years of nursing experience (hospital preferred).
