Job summary

UP Health System - Marquette is seeking a dedicated RN Case Manager to oversee patient care coordination, discharge planning, and utilization management in a high-acuity Acute Care setting in Marquette, Michigan.

You will collaborate with physicians, nursing staff and care teams to ensure timely discharges, appropriate level of care, and optimal patient outcomes while maintaining compassionate, patient-centered service across the hospital.

Qualifications

• Current Michigan RN license
• Associate's degree from an accredited nursing program (Bachelor's preferred)
• BLS Health Care Provider certification (BLS-HCP)

Responsibilities

• Monitors and manages patient care to promote continuity of care and optimal outcomes.
• Consults with nursing staff and multidisciplinary team on status, admission, LOS, transfer and discharge.
• Monitors patient and family satisfaction; addresses questions/complaints from patients, families and payors.
• Participates in discharge planning and coordinates transfers to other facilities.
• Performs chart reviews to identify issues with service delivery, outcomes, compliance, cost and reimbursement.
