Job summary

The position requires a BSN or ADN with BSN to obtain within 4 years, Michigan RN license, and preferred Case Management certification. Prior 2+ years in acute care case management is expected.

Qualifications

• RN Case Manager position in an acute hospital setting requiring nursing licensure in Michigan.
• Minimum BSN or ADN with BSN to be obtained within 4 years of hire; current MI RN license.
• Preferred certification in Case Management or related professional certification.

Responsibilities

• Provide competent patient assessments and data interpretation to guide care plans.
• Assess discharge needs and coordinate care coordination across the health continuum.
• Manage cases per organizational standards and promote effective resource utilization.
• Monitor and adjust plans through collaboration with the care team, patient, and family.
• Engage in active listening and empathetic communication with patients and families.
• Coordinate with physicians, nurses, and community resources to plan delivery and discharge.
• Promote collaborative relationships and advocate for patients and families.
• Educate patients and families about care options and follow-up plans.
