Job summary

Center for Elders’ Independence is seeking an RN Case Manager (CCSC) to coordinate care and oversee transitions for CEI participants in hospital and SNF settings. The role emphasizes resource management, collaboration with clinicians, and ensuring appropriate levels of care and timely disposition.

The CCSC RN Case Manager works with an interdisciplinary team to optimize patient outcomes, monitor care, and support discharge planning while navigating CEI benefits and contract requirements.

Qualifications

• Requires a California RN license.
• 3–5 years of case management experience are required.
• BSN in Nursing is required from an accredited school.

Responsibilities

• Assess and evaluate care for CEI members in non-CEI settings.
• Develop and evaluate treatment plans with providers.
• Authorize hospital or SNF stays in a timely manner.
• Serve as liaison with hospital during business hours.
• Collaborate with physicians and IDT for optimal discharge.
