As a Hospice RN, your responsibilities include:

• Conducting initial nursing evaluations within 48 hours of referral to determine hospice eligibility
• Assessing and re-assessing patients' physical, psychosocial, and environmental needs, at least every 15 days
• Initiating and coordinating the care plan
• Documenting problems, goals, interventions, and patient/family responses
• Collaborating with patients, families, physicians, and the interdisciplinary team
• Teaching and supervising patients and families in self-care techniques
• Maintaining accurate clinical notes

Experience and License Requirements

• Minimum of one year of clinical nursing experience
• Current RN licensure in the practicing state
• Valid CPR certification
• Valid driver’s license, insurance, and reliable transportation or access to public transit
