Responsibilities include:

• Making the initial nursing evaluation within 48 hours of referral to determine eligibility for hospice services.
• Identifying and reassessing the patient's and family's physical, psychosocial, and environmental needs at least every 15 days.
• Initiating and coordinating the plan of care.
• Documenting problems, goals, interventions, and responses to care.
• Collaborating with the patient, family, attending physician, and other team members daily.
• Instructing and supervising the patient and family in self-care techniques when appropriate.
• Maintaining accurate clinical notes regarding the patient's condition.

Experience Requirements:

• At least one year of clinical experience.

License Requirements:

• Current RN licensure in the state of practice.
• Current CPR certification.
• Valid driver's license, insurance, dependable vehicle or access to public transportation.
