Job summary

Southcentral Foundation is seeking a Registered Nurse to join the RN Case Manager Home Visit Team in Anchorage, AK. The role delivers skilled nursing services and care coordination to customer-owners who are homebound or unable to access clinic care.

In this position you will perform wound care, clinical assessments, medication support, and collaborate with primary care teams to manage complex medical needs in home and community settings while adhering to Native Preference policies.

Qualifications

  • RN required with Alaska license.
  • BLS certification required; ACLS/NRP/PALS as needed for specialty.
  • One year nursing experience or completion of the RN Case Manager Training Program at SCF.

The Southcentral Foundation RN Case Manager Home Visit Team provides timely, appropriate, and beneficial nursing care to customer-owners who are homebound or otherwise unable to access care in the clinic setting.

This position serves as an extension of the primary care team by providing skilled nursing services, care coordination, wound care, clinical assessments, medication support, and communication with primary care teams to support customer-owners with complex medical needs in the home and community-based setting.

Qualifications:

SCF programs are established to serve a primary population comprised of Alaska Native people who are affiliated with Cook Inlet Region, Inc. (CIRI) and Alaska Native and American Indian people within SCF’s geographical service area. Employees should have a thorough understanding of the cultures and the needs of this population. Such knowledge is critical to ensure the achievement of SCF’s vision of a Native Community that enjoys physical, mental, emotional, and spiritual wellness, and mission of working together with the Native Community to achieve wellness through health and related services:

  • Graduate of an accredited school of nursing.
  • License in the State of Alaska as a Registered Nurse.
  • Basic Life Support (BLS) certification is required. ACLS, NRP, ATLS, ALSO and PALS as required for specialty.
  • One (1) year nursing experience or successful completion of the RN Case Manager Training Program at SCF.
