Job summary

Angels Care Home Health in Shenandoah, IA seeks a Registered Nurse Case Manager to coordinate all aspects of a patient’s home care, perform admission visits, and determine eligibility for services.

You will collaborate with physicians, therapists, and social workers to develop and implement individualized plans of care, monitor progress, adjust treatments, and ensure safety and quality for diverse home health patients.

Qualifications

• Graduate of an accredited nursing program (diploma/ADN/BSN).
• Active RN license in Iowa.
• Valid driver's license and reliable transportation.
• CPR certification and Hepatitis vaccination/declination on file.

Responsibilities

• Coordinate all aspects of home care for patients.
• Perform admission visit and determine eligibility for home care services.
• Develop and monitor individualized plans of care with interdisciplinary team.
