Job summary

Angels Care Home Health is seeking a Registered Nurse Case Manager to coordinate comprehensive home care, collaborating with medical teams to deliver quality patient care in the home setting. You will assess eligibility, develop and implement the plan of care, and ensure smooth admission visits with professional nursing standards.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver's License.
• One-year experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• The RN Case Manager coordinates all aspects of the patient's home care.
• Interact with other medical disciplines as needed and provide professional care.
• Perform admission visit to determine eligibility and develop the plan of care.
