Job summary

Angels Care Hospice in Iowa seeks a Registered Nurse Case Manager to coordinate all aspects of the patient’s home care. You will collaborate with physicians, therapists, and family members to implement a comprehensive plan of care in the patient’s home.

This role requires a nursing degree, current RN license, and a valid driver's license; prior RN home health experience is valued. We offer paid time off, health benefits, and opportunities for advancement.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver's License.
• One-year's experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient's home care.
• Interact with other medical disciplines as needed.
• Perform the admission visit to the patient in the home.
• Determine the patient's eligibility for home care services.
• Develop the plan of care to be followed.
