Job summary

Angels Care Home Health in Missouri is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care, perform admission visits, determine eligibility for home care services, and develop the plan of care.

Qualifications include RN license, driver’s license, CPR, Hepatitis consent/declination, and at least 1 year of RN experience (2 preferred). We offer competitive salary, vehicle allowance, generous PTO, and benefits.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Valid driver’s license and reliable transportation.
• One year’s RN experience; two preferred.
• CPR certification and Hepatitis consent/declination.
• Auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Perform the admission visit in the home.
