Job summary

Angels Care Home Health is seeking an RN Case Manager who will coordinate all aspects of the patient’s home care and interact with other medical disciplines to deliver professional care. You will perform the admission visit to determine eligibility for home care services and develop the plan of care to be followed.

Requirements include an active RN license, CPR proof, a valid driver’s license, and reliable transportation. Experience in home health is preferred.

Qualifications

• Current state RN license is required.
• CPR certification required or proof of CPR.
• Valid driver's license and reliable transportation.

Responsibilities

• Coordinate all aspects of patient home care with care team.
• Perform admission visit to assess eligibility for home care.
• Develop and monitor the plan of care for each patient.
• Collaborate with other medical disciplines as needed.
