Job summary

Angels Care Home Health is seeking a Provider Support Registered Nurse (RN) Case Manager to coordinate patient home care, interact with other medical disciplines, and perform admission visits in the home. You will determine eligibility for services and develop the plan of care for each patient.

This full-time role requires an active RN license, 1+ year of RN experience, CPR, Hepatitis consent/declination, and willingness to travel across branches within the provider.

Qualifications

• Graduate of an accredited diploma, Associate, or Baccalaureate School of Nursing.
• Current state RN license.
• One year RN experience (two preferred).
• CPR certification and Hepatitis consent/declination.
• Willingness to travel to all branches within a provider.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed.
• Perform admission visit to patient in the home.
• Determine eligibility for home care services.
• Develop the plan of care to be followed.
