Job summary

Angels Care Hospice is seeking a Registered Nurse Case Manager to coordinate all aspects of in-home patient care. You will collaborate with medical teams, perform admissions visits, assess eligibility for home care services, and craft individualized plans of care for patients in their homes.

You should have an accredited nursing degree, an active RN license, and a valid driver’s license, with CPR/Hepatitis consent documentation and reliable transportation.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver's License.
• One-year's experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• The Registered Nurse (RN) Case Manager coordinates all aspects of the patient's home care.
• Interact with other medical disciplines as needed and provide professional care to the patient.
• Perform the admission visit to the patient in the home, determine eligibility for home care services, and develop the plan of care to be followed.
