Job summary

Angels Care Home Health in Newton, IA is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care. You will perform admission visits, determine eligibility for home care services, and develop the plan of care to be followed in collaboration with the care team.

The role requires an active RN license, a valid driver’s license, CPR and Hepatitis consent/declination, and reliable transportation with auto liability insurance.

Qualifications

• Graduate of an accredited nursing program (diploma/ADN/BSN).
• Current state RN license.
• Valid driver’s license.
• 1 year RN experience (2 preferred).
• CPR certification and Hepatitis consent/declination.
• Reliable transportation with auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care as RN Case Manager.
• Perform admission visit to the patient in the home and determine eligibility for home care services.
• Develop the plan of care to be followed with input from other medical disciplines as needed.
• Provide professional nursing care to the patient and communicate effectively with the care team.
