Job summary

Nightingale Home Healthcare of Minnesota Inc. is hiring Registered Nurse Case Managers for our Minnetonka office. This PAY PER VISIT role offers a pathway to substantial earnings through patient visits.

The position requires MN RN licensure, 1+ year in medical/surgical or home health, and CPR. Mileage reimbursement, 401K match after one year, and comprehensive benefits accompany flexible scheduling.

Qualifications

• Must be licensed Registered Nurse in MN.
• Minimum 1 year medical/surgical or home health experience preferred.
• WellSky (Kinnser) experience preferred.
• Current TB test within 12 months.
• Current CPR Certification.
• Valid driver’s license and proof of liability insurance on automobile.
• Ability to attend 3-day orientation at our Minnetonka office.
• Ability to work 8 AM – 5 PM Monday – Friday with one weekend a month on call.

Responsibilities

• Obtains medical history from patient/family.
• Conducts brief physical examination including vitals.
• Evaluates home situation to determine health teaching needs.
• Assesses services needed (LPN, HHA, PCA) and frequency.
• Interprets nursing services to patient and family for care planning.
• Develops and implements nursing care plans.
• Provides skilled nursing care per the plan.
