Job summary

Home Health Care, Inc. is seeking a dedicated Registered Nurse (RN) Case Manager to build lasting patient relationships across Bloomington and Eden Prairie, MN. You will manage home health caseloads with on-call and weekend visits within ~35 miles of home.

You'll complete OASIS assessments, develop individualized plans of care, deliver skilled nursing care, coordinate with physicians and the interdisciplinary team, and document meticulously to support patient goals and safety at home.

Qualifications

• Active Minnesota RN license.
• 3+ years RN experience (home health preferred).
• Ability to travel locally.

Responsibilities

• Manage a home health caseload in Bloomington and Eden Prairie, with on-call and weekend visits within ~35 miles.
• Complete OASIS assessments and individualized plans of care.
• Deliver skilled nursing care and patient education.
• Coordinate care with physicians and interdisciplinary team.
• Help patients achieve their goals while remaining safely at home.
• Maintain timely and accurate documentation.
