Job summary

Nightingale Home Healthcare is seeking a skilled Registered Nurse (RN) to deliver direct patient care and case management in home health settings in Minnetonka, MN. You will conduct assessments, develop care plans, and coordinate with the care team to ensure quality outcomes.

Qualifications

• Active RN license in MN is required.
• Experience with OASIS and WellSky (Kinnser) strongly preferred.
• Med-surg and wound care experience is a plus.

Responsibilities

• Provide direct patient care and case management.
• Conduct patient admissions and develop care plans.
• Coordinate care with interdisciplinary team and document visits promptly.
• Educate patients and families on home health services.
