Job summary

Nightingale Home Healthcare of Minnesota Inc in Minnetonka, MN is seeking a passionate Registered Nurse (RN) to provide direct patient care and case management in home health, developing care plans and coordinating with the interdisciplinary team.

You will perform admissions, conduct assessments, educate patients and families, deliver skilled nursing care, and ensure HIPAA compliance while documenting visits promptly.

Qualifications

• Active MN RN license.
• Experience with OASIS and WellSky (Kinnser) preferred.
• Med-surg and wound care experience a plus.
• Current TB test within last 12 months.
• CPR certification.
• Valid driver’s license.
• Attend orientation at Minnetonka office.

Responsibilities

• Obtain medical history and assess patient condition and needs.
• Conduct physical assessments, including vitals and mobility evaluations.
• Educate patients and families on care plans and home health services.
• Deliver skilled nursing care while maintaining HIPAA compliance.
• Document and submit accurate visit records promptly.
• Develop and update individualized care plans as needed.
• Coordinate with LPNs, Home Health Aides, and Personal Care Attendants.
• Facilitate referrals for therapy or medical social work.
• Evaluate and adjust nursing services as appropriate.
