Job summary

Home Health Care, Inc. in Minnesota is seeking an experienced Registered Nurse (RN) Case Manager to deliver high-quality home health care in Burnsville and surrounding areas.

This full-time role offers a Monday–Friday schedule with one weekend per month and one on-call shift per week. You will conduct comprehensive assessments, develop care plans, coordinate with physicians and the clinical team, and support patients to stay safely at home.

Qualifications

• Active Minnesota RN license.
• 3+ years of RN experience (home health preferred).
• Strong assessment and critical thinking skills.
• Excellent communication and organizational abilities.
• Reliable transportation for local travel.

Responsibilities

• Manage a home health caseload across Woodbury, Cottage Grove, Hastings, St. Paul Park, and Landfall.
• Complete comprehensive OASIS assessments and individualized care plans.
• Deliver skilled nursing care and patient education.
• Collaborate closely with physicians and interdisciplinary team members.
• Help patients recover, remain independent, and avoid unnecessary hospitalizations.
• Maintain accurate clinical documentation.
