Qualifications

• RN license in MN required.
• OASIS experience preferred.
• Wellsky (Kinser) experience preferred.

Responsibilities

• Obtain medical history from patient/family.
• Conduct brief patient examinations and record vitals.
• Provide skilled nursing care and maintain patient confidentiality.
• Coordinate and supervise care with LPNs/HPAs/PCAs.
• Document visits and update patient records regularly.
