Job summary

Nightingale Home Healthcare of Minnesota Inc in Minnetonka, MN is seeking full-time/part-time Registered Nurse Case Managers to deliver skilled nursing care at patients' homes under physician orders and the Director of Clinical Services.

RNs administer care, educate families, and coordinate with LPNs and aides while following the plan of care. The pay-per-visit model offers substantial earning potential, with visits starting around $60 each and opportunities for merit-based growth.

Qualifications

• Licensed Registered Nurse in MN.
• 1 year medical/surgical or Home Health (OASIS) or geriatric nursing experience preferred.
• Wellsky (Kinnser) experience preferred.
• Current TB results (within last 12 months).
• Current CPR Certification.
• Valid driver’s license and proof of liability insurance on automobile.
• Ability to attend 3-day orientation at our Minnetonka office.
• 8 AM – 5 PM Monday – Friday with one weekend a month on call.

Responsibilities

• Provide skilled nursing care per physician orders and plan of care.
• Coordinate and supervise care by LPN/Home Health Aide/Personal Care Attendant.
• Educate and supervise family members regarding nursing care needs.
• Maintain confidentiality of patient information per HIPAA regulations.
• Communicate with the Director of Clinical Services to adjust care plans as needed.
