Job summary

Redcrest Home Health in Indiana is seeking a Registered Nurse Case Manager (RNCM) to lead patient assessments and care planning. The RNCM coordinates multidisciplinary teams, updates orders, and ensures compliant documentation across the care continuum.

The role requires strong communication, clinical expertise in home health, and the ability to educate patients and families. A valid RN license and reliable transportation are required; a $10,000 sign-on bonus is available.

Qualifications

• Minimum of one year of RN experience in an acute care setting; home care desired.
• Strong communication with patients, families, physicians and co-workers.
• Ability to remain calm, patient and caring while working with patients.

Responsibilities

• Oversees overall patient care from start to end of episode.
• Updates doctor orders in patient chart.
• Notifies doctor of significant condition changes.
• Manages multi-disciplinary care.
• Provides skilled nursing care in patients' homes as ordered by physician.
• Documents all aspects of treatment and education.
• Completes clinical documentation per agency protocol and Medicare/ Federal guidelines.
