Job summary

Team Select Home Care in Anderson, IN seeks a qualified registered nurse to serve as Case Manager, overseeing patient care plans and coordination with physicians and staff. You will evaluate needs, manage data collection, and document care in accordance with state and payer requirements.

Responsibilities include developing care plans, supervising aides, administering medications per policy, and communicating with families.

Qualifications

• Graduate of an approved school of professional nursing and licensed in the state(s) where practicing.
• Minimum 1 year nursing experience; home health experience preferred.
• Ability to organize work, supervise ancillary staff, and follow physician orders.

Responsibilities

• Develops and evaluates patient-centered care plans under physician supervision.
• Provides case management for an assigned group of patients and staff.
• Documents data sets (OASIS, PAR) and other required records.
• Coordinates care among patients, families, and providers.
• Performs nursing procedures and medication administration per policy.
