Job summary

Team Select Home Care is seeking a Registered Nurse for a Case Manager role in Indiana. The RN will oversee patient care plans, coordinate with families and healthcare providers, and ensure documentation like OASIS and PAR are submitted timely.

The role requires at least 1 year of nursing experience (home health preferred) and valid RN licensure. Duties include case conferences, medication reconciliation, and on-call coverage as needed.

Qualifications

• Graduate of an approved nursing program and currently licensed in the practicing state(s).
• Minimum 1 year nursing experience; home health preferred.

Responsibilities

• Develops, implements, and evaluates individualized patient-centered care plans under physician supervision.
• Provides case management to an assigned group of patients and staff.
• Communicates changes in patient condition to the physician and care team.
