Job summary

Adara Home Health, Inc. is seeking an RN Case Manager to provide direct client care and oversee home health services in the Minnesota community. You will collaborate with families and the health team to develop goal-centered care plans while considering quality measures and treatment costs.

The role requires 1:1 client visits in homes, travel within the Buffalo, MN region, and on-call responsibilities. A valid MN RN license is mandatory.

Qualifications

• Active Minnesota RN license in good standing.
• Two years of healthcare or home care experience.
• Ability to travel to client homes across service area.

Responsibilities

• Perform initial and ongoing client assessments and plan care.
• Provide skilled nursing care and clinical oversight.
• Collaborate with health team to update care plans every 60 days.
• Ensure safety, documentation accuracy, and payer/billing compliance.
• Educate clients, families and caregivers.
