Job summary

Adara Home Health, Inc. is seeking an RN Case Manager to deliver direct client care and oversee home health services within the community.

You will collaborate with families and the health care team to create and update goal-centered care plans, considering quality measures and costs of treatment. You will travel to client homes for 1:1 care, manage the schedule during office hours, and participate in on-call rotation.

Qualifications

• Minnesota RN license in good standing.
• Proof of negative TB testing.
• Access to a dependable automobile to travel between client sites.
• Valid driver’s license and current car insurance.
• US Citizenship or valid Alien Work Permit.
• Background checks including MDH and OIG lists.
• Disclose any criminal history relating to health services.

Responsibilities

• Reflects the company’s mission and values in professional relationships.
• Performs initial and periodic assessment of clients to determine care needs and evaluates outcomes.
• Collaborates with physicians and home health team to update the care plan every 60 days.
• Serves as client advocate, addressing safety and abuse prevention.
• Maintains documentation accuracy and compliance with payer requirements.
• Provides education to clients, families, caregivers, and staff.
• Provides direct care to clients per policies and Minnesota Nurse Practice Act.
• Coordinates client care with family and community resources.
• Provides clinical oversight and leadership to staff.
• Ensures effective financial management of home health services.
• Manages schedule and participates in on-call process.
