CVS Health is seeking an experienced RN Case Manager in Massachusetts to join our Community Care team. You will collaborate with members, providers, and community organizations to address health care and social needs, coordinating case management across the continuum of care.

The role emphasizes compassionate communication and precise documentation. Required MA RN licensure and 3+ years of clinical practice, plus 2+ years in case management or home health.

• Active and unrestricted MA RN licensure.
• 3+ years of clinical experience in relevant fields.
• 2+ years in case management, discharge planning, or home health coordination.
• Proficient with MS Office and required applications.
• Strong time management and problem-solving abilities.
