CVS Health in Massachusetts is seeking a Nurse Case Manager to coordinate comprehensive care, assess member needs, and develop individualized care plans. You will liaise with providers, insurers, and families to ensure optimal outcomes and timely care.

The role requires MA RN licensure, 3+ years clinical experience with Medicare populations, and CM or discharge planning experience. Remote work within MA allowed; strong communication and documentation skills are essential.

Competenze

• Active MA RN licensure
• 3+ years clinical experience (diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac, Home Health)
• 2+ years case management or discharge planning/home health coordination
• Proficiency with MS Word, Excel, Outlook, PowerPoint
• Able to work in a fast-paced high-volume environment; strong time management
• Working knowledge of medical terminology and digital literacy

• Coordinate and facilitate comprehensive care for individuals by assessing needs and developing personalized care plans.
• Coordinate services across healthcare providers and advocate for patients.
• Interact with members telephonically or in person; may visit homes, worksites or physician offices.
