CVS Health is seeking a Case Manager to coordinate comprehensive care for members, assess needs, and develop personalized care plans. You will advocate for patients, ensure effective communication with providers and insurers, and monitor progress to promote positive outcomes.

You will interact with members telephonically or in person, document activities, and collaborate with an internal team to optimize wellness and return to baseline status.

Qualifikationen

• RN licensure in resident state (PA, OH, FL, NJ, GA, NC, TX, CT) is required.
• 3+ years of clinical practice experience preferred in diabetes, CHF, CKD, post-acute, hospice or related areas.
• 2+ years of case management, discharge planning or home health coordination.
• Proficiency with Microsoft Office and basic proprietary applications.

• Coordinate and facilitate comprehensive care for individuals and develop personalized care plans.
• Act as advocate, ensuring effective communication and resource use across providers and plans.
• Document case activities accurately and monitor progress toward optimal outcomes.
• Liaise with members, families, employers, providers and insurers as appropriate.
• Support education and prevention information for best medical outcomes.
• Navigate multiple systems to support timely return to baseline and appropriate care levels.
