CVS Health is seeking a Community Care Case Manager to coordinate case management for high-need members across the care continuum. The role involves assessment, planning, advocacy, and collaboration with providers, insurers, and employers to promote quality health outcomes.

Requires RN licensure with 3+ years clinical experience (diabetes, CHF, CKD, post-acute, hospice, palliative, cardiac) and 2+ years in case management or home health.

Qualifications

• Must reside in CVS South Region: OH, VA, NC, KY, TN, SC, GA, AL, MS, AR, LA, FL.
• Active, unrestricted Compact RN license in OH or in home state.
• 3+ years clinical practice (diabetes, CHF, CKD, post-acute, hospice, palliative, cardiac) with Medicare.
• 2+ years case management, discharge planning or home health coordination.
• Bilingual - Spanish (Preferred).
• Strong analytical, problem-solving, and interpersonal skills.
• Willing to obtain multi-state RN licenses; CVS will provide certification.

Responsibilities

• Coordinate case management activities for catastrophic and chronically ill members across care continuum.
• Interact with members telephonically or in person and document case work.
• Assess needs, develop care plans, and facilitate benefits eligibility.
• Collaborate with internal multidisciplinary teams to optimize outcomes.
• Comply with HIPAA, regulations, and employer policies.
