• Strong communication and interpersonal skills.

Responsibilities

• Coordinate and facilitate comprehensive care for patients.
• Advise and communicate with member/family, providers, and insurers.
• Plan and monitor case management across continuum of care.
• Conduct evaluations of needs and eligibility for benefits.
• Maintain documentation of case work activities.
• Collaborate with internal multidisciplinary teams.
• Engage home visits or telephonic outreach as needed.
• Ensure compliance with laws and company policies.
• Support return to baseline and optimal functioning.
