Job summary

CVS Health is seeking a dedicated RN Case Manager in Massachusetts to coordinate comprehensive care for members, develop care plans, and communicate with providers and families. The role focuses on monitoring progress, advocating for patients, and facilitating efficient service utilization within a home-based/remote framework.

This position requires MA RN licensure and several years of clinical and case-management experience, with a preference for candidates with a Bachelor's in Nursing.

Qualifications

• Active and unrestricted RN licensure in MA.
• 3+ years of clinical practical experience (diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac, Home Health).
• 2+ years of case management, discharge planning and/or home health care coordination experience.
• Proficiency with Microsoft Word, Excel, Outlook and PowerPoint; ability to navigate multiple systems.

Responsibilities

• Coordinate and facilitate comprehensive care for individuals across providers.
• Assess needs, develop personalized care plans, and coordinate services.
• Interact with members telephonically or in person.
• Prepare documentation of case work activities and maintain records.
• Ensure regulatory compliance and optimal wellness outcomes.
